Community health centers use a nationally recognized health care model that provides high-quality primary care to all patients served regardless of insurance coverage or income status.

The wide variety of services offered allows community health centers to provide the consistent care that makes each clinic site a health care home for all who access services.

There are currently 14 community heatlh centers in North Dakota and 34 community health centers in South Dakota. Community health centers offer a sliding fee scale to patients who meet the income requirements.
