The Sliding Fee Scale (SFS) program offers individuals and families an option to gain access to quality, medical, dental, and prescription drug services. The individual or family must meet eligibility requirements established through the community health center (CHC) sliding fee policy.
The patient must submit a completed SFS application every year and may qualify for a discount depending upon their household size and gross income. Those eligible receive a discount to the cost of most, but not all, services provided by CHC health professionals.
