Community health centers (CHCs) start with the community. With a focus on high quality, affordable primary and preventive health care, the goal of each CHC is to become the medical home for the patients they serve. Each site provides behavioral, medical, and enabling services such as language translation and case management services. Some clinic sites also offer dental and pharmaceutical services.
